Community Health Needs Assessments

Covenant Health is a mission-driven healthcare enterprise committed to improving the quality of life in our region through better health. We are dedicated to being the region’s premier healthcare network through service, community and innovation.

As a not-for-profit health system, our member hospitals complete data-driven Community Health Needs Assessments (CHNA) every three years. These assessments evaluate the overall health status of the areas we serve and include input from individuals and organizations representing the broad interests of our communities. Their feedback, in combination with socioeconomic data, helps Covenant Health identify and prioritize community health needs, particularly for underserved and vulnerable populations.

In addition to the assessments, our hospitals also develop Implementation Plans that outline action steps and collaborative resources for each facility to help address specific community health needs.
